CLINICAL
EXPERIENCE
At the Los Robles Hospital (1998-present) I have
performed 600-800 procedures per year involving Coronary
Angiography,Coronary Angioplasty, coronary stent
placement and, Atherectomy/rotoblater, intravascular
ultrasound, intracardiac ultrasound, fetal subclavian
and peripheral angioplasty and stent placement.
Pioneered new procedures involving closure of the PFO/ASD
and performance of carotid angioplasty and stent
placement using distal protection devices.
I have been a proctor for coronary stenting and have
visited about 20 hospitals for training about 32
interventional cardiologists in the optimal placement of
coronary stenting.
At the VA Medical Center as Director of Cardiac Cath
Lab( 1995-98) I performed complex interventional
procedures in some very high risk patients many of
whom were considered inoperable, with good outcomes.
Areas of special interest and expertise: Coronary
stenting; Rotoblator; Renal, subclavian and peripheral
angioplasty; Intravascular ultrasound.
Carotid Angioplasty and Stent placement with distal
protection devices.
Percutaneous Closure of PFO/ASD.
At Cedars-Sinai Medical Center, I underwent a training
in advanced interventional procedures including excimer
laser angioplasty, directional atherectomy, placement of
Gianturco-Rubin and Palmaz-Schatz stents and arterial
coil embolization. In July, 1994, I joined Drs. Litvack
and Eigler as the third full-time staff in the Cath Lab.
Overall, I have about 20 years experience in the field
of interventional cardiology (1985-1997; including
training period) in a diverse array of procedures for
coronary artery disease, valvular heart disease,
peripheral vascular and venous occlusive disorders. The
experience includes state-of-the-art technology as an
independent operator.
During 4-1/2 years (1988 - July, 1992) of work as an
Assistant Professor at AIIMS focused on interventional
cardiology. Performed more than 1200 diagnostic cardiac
catheterization procedures (right and left heart
studies, coronary angiography). Assisted in about 300
coronary angioplasty procedures and independently
performed 60 PTCA procedures. Performed balloon
valvoplasty of the mitral valve in rheumatic mitral
stenosis in 80 patients, pulmonic stenosis in 40 cases
and aortic stenosis (in the young) in 25 cases. Started
Inoue balloon technique for mitral valvoplasty at our
center and started balloon angioplasty of the inferior
vena cava and performed this procedure in 32 cases of
IVC obstructions.
During 3 years
of my Fellowship in Cardiology (January, 1984 to
December, 1987) at AIIMS, New Delhi-India, I was duly
trained in all areas of clinical cardiology, cardiac
catheterization and angiography, interventional
cardiology, echocardiography and other noninvasive
diagnostic procedures, management of cardiac emergencies
and coronary care.

RESEARCH
I. Research at VA Medical Center:
1. Evaluation of Endotoxin antagonist in prevention of
acute renal failure in patients with CRF and creatinine
of >2.0 mg induced by contrast medium used for
coronary angiography. A multicenter randomised clinical
trial. Sponsored by Smith Kline &Beecham.
2. Comparative evaluation of the ACT-one nitinol
coronary stent and J&J stainless steel stent. A
multicenter randomised clinical study, sponsored by
Progressive angioplasty systems.
3. Evaluation of xemlofiban in preventing thrombotic
events after coronary angioplasty and stents. Sponsored
by GD Searle and Co.
4. The VA cooperative trial of PTCA vs CABG in high risk
patients with unstable angina. A randomised controlled
trial.
5. Evaluation of a Na+/H+ exchange inhibitor (cariporide)
in preventing acute MI and mortality in patients with
acute coronary syndromes and after coronary angioplasty.
A double blind placebo controlled multi center trial.
6. Evaluation of intracoronary ultrasound and Doppler
Flow measurement in assessing coronary stenosis before
and after PTCA/coronary stenting. In preparation.
II. Research at Cedars-Sinai Medical Center:
Focus on prevention of restenosis after angioplasty in
animal models.
a) Local Drug Delivery:
1. Via polyurethane coated removable stent: studied the
kinetics of drug delivery of lipophilic drug etretinate
to the vessel wall and demonstrated prolonged delivery
of high concentrations of the drug to the wall for up to
1 week. Published 2 papers and 5 abstract (in ACC/AHA).
2. Drug containing PLA microspheres: demonstrated safety
and prolonged persistence of rhodamine loaded
microspheres in the vessel wall after delivery via the
porous balloon. Studies on prevention of intimal
hyperplasia after arterial injury using dexamethasone,
colchicine and probucol loaded in PLA microspheres are
being done. Presented 4 abstract (AHA & ACC), Paper
in Press.
3. Polyvinyl alcohol coating for the stent: for
hydrophilic drugs (heparin, hirulog, angiopeptin,
oligonucleotides) I have developed a PVA coating for the
stent. Studies of drug incorporation and release are
being performed.
b) Molecular Biology:
1. Ribozyme project: Use of chimeric hammerhead ribozyme
to cdc-2 kinase & PCNA to prevent intimal
hyperplasia in a rat carotid artery model.
a) In-vitro: study of effect of CDC-2 kinase, PCNA &
DCD 2-kinase ribozymes rat on smooth muscle cell
proliferation. Measurement of (ii) translation of the
target protein by S 35 methione uptake; target protein
levels by western blotting and immunoprecipitation (i)
transcription of MRNA for target protein by northern
blotting and RT PCR. Studies in progress, encouraging
results.
b) In vivo: Study of local delivery of CDC-2 kinase and
PCNA ribozyme to the rat carotid artery after balloon
injury. Quantitative histology at 2 weeks to study
reduction in intima-media ratio (completed). Study of
target protein translation and transcription of MRNA for
the target protein in arterial tissues (ongoing).
2. Use of antisense oligonucleotide to cdc-2 kinase to
prevent SMC proliferation in-vitro. Plan to use cdc-2
kinase and PCNA antisense ON in in-vivo studies after
standardization of delivery systems.
3. Ph Sensitive Liposomes: For enhanced intracellular
delivery of oligonucleotides (and other drugs) we are
evaluating Ph sensitive liposomes. A comparative study
of efficacy of delivery of fluorescine labeled ON by Ph
sensitive liposomes, traditional liposomes or free
oligonucleotides in the presence or absence of
chloroquine is being done.

a) Clinical research in Cardiology: In Interventional
Cardiology, echocardiography and other areas. Published
170 papers in various Journals including 9 chapters in
books, 10 reviews, 35 reports and 65 abstracts of the
papers presented in various conferences.(List attached.)
b) Guidance to Postgraduate Research: During 4-1/2 years
of my work as an Assistant Professor in Cardiology at
AIIMS, I was on the Doctoral Committee for the PhD
thesis on hemodynamics of portal hypertension of a
postdoctoral fellow in GI surgery. I was a co-guide for
the DM thesis of 3 Fellows in Cardiology, 2 Fellows in
endocrinology and for the MD thesis of 2 postgraduate in
internal medicine.
